FARGO DIRECTIONS:
The office is located at 650 1st Avenue North, Suite 114, (note: this is not the mailing address) in the Gardner building on the corner of Roberts Street and First Avenue North. Enter the building using the main entrance facing 1st Avenue (facing the federal building). Upon entering the building lobby, the suite is located to your right just beyond the salon. Parking is available in the parking lot located on the west side of the building. The first five spots in the first row are reserved for customers. These spots are marked and easy to identify. On street parking is usually available on the east and north side of the building.
VALLEY CITY DIRECTIONS:
The office is located at 136 2nd Avenue NE, directly across from Taco Johns. Enter the building using the Pain Relief Center entrance. To protect your privacy our business sign is not posted on the door, but you will see our sign on the window to your left. Please wait in the lobby. A counselor will greet you at the time of your appointment. Parking is available on the street and parking lot.
WHAT TO BRING:
If you have insurance, please bring your insurance card to your appointment. Intake forms are available here or you can complete the form at the time of your appointment.
FEES:
Adult Program: fee for individual counseling is $100.00 per session. You are responsible for copays and any services not covered under your insurance plan.
Co-pay fees are collected at the time of your visit
Couples/Marriage Program: fee for couples counseling is $70.00 per hour. Most insurance plans do not cover marital therapy or couples counseling, but we would be happy to provide you with an invoice for your own submission efforts. Rates are lower to support couples of all income levels. Payment will be collected at the time of your visit.
We accept most insurance options, cash, checks and credit cards.
Please give 24 hours advance notice when canceling an intake appointment.
